Sign Up for CHSM Events or Membership
	CHSM offers several ways to register to attend an event, or sign up for CHSM membership:
	Register Online Use our secure online registration to register and pay online -- or register online and pay at the door.  Questions about online registration?       Register by Phone, Fax, Mail, or Email Send registration information via phone, or . Include name(s), title, organization, address, phone, email, and type of membership if applicable.  Or, download a registration form and send it via fax, or mail.
	Program Registration
	Pre-Register:  Please pre-register for the lowest cost, and to reserve a space for yourself at our limited-seating events. Late and at-the-door registrations are accepted on a space available basis. The cut-off for pre-registration is one week before the program.  For example, pre-registrations for the April 28th program must be received by midnight on April 21st.  All registrations made during the week before the program are considered late registrations. Register Online!
	Registration Fees:   The registration fee is set to cover the cost of the facility and meal expenses.

	Register Online ! Payment: Event seating is limited and facility commitments are made for registered attendees. No refunds are given for non-attendance. Outstanding registration balances will be billed. Cancellations are accepted until 7 days before the event, the pre-registration deadline. After the pre-registration deadline, you are welcome to transfer your registration to a substitute.
	CHECK • Make checks payable to CHSM - Colorado HealthCare Strategy & Mgmt.  • Bring payment to the event, or mail to:          CHSM          423 West First Street          Minden, NE 68959-1509  (Why does a Colorado association have a Nebraska address?)        CREDIT CARD • Register & Pay online with secure credit card transactions! • Visa and Mastercard accepted at the door.
	Cancellation/ Substitution / Refunds / No-Shows:  A registration for a CHSM event indicates a commitment to pay.  If you are unable to attend the program after you have registered, you may cancel your registration for full credit up to one week before the event (for example, by April 21st for the April 28th breakfast meeting).  If you have already paid for the program, we will hold your registration fees for a future program, or, at your request, we will issue a credit based on your form of payment (to your credit card or through a refund check, as appropriate). Transfer your registration if you have to cancel less than one week before the program. We regret that we cannot credit your registration fees because CHSM has already paid for your breakfast.  However, you may transfer your registration to another person.  If you alert the CHSM Administrator of the substitution three or more days before the event, we will prepare a name tag for the person actually attending the program. For cancellations less than one week before the program date, as well as program no-shows, CHSM will collect unpaid registration fees.
	Remember: • Pre-registration deadlines are one full week prior to the event. • Register by pre-registration deadline to get the lowest price. (And for the very lowest price plus year-long savings, become a CHSM member!) • Seating is limited.  Space cannot be guaranteed if you do not pre-register. • All registrations indicate a commitment to pay. • No-shows and cancellations less than one week before the event require full payment.
